Clinical Site Prerequisites: Graduate Students

Everything listed for your assigned clinical site MUST be completed prior to beginning your rotation. Failure to do so may cancel your clinical placement.

  1. Download and complete any forms and/or training.
  2. Completed forms must be returned according to the timeline indicated in the site links below. CHECK THE REQUIRED DEADLINE FOR EACH SITE, as some sites require forms to be submitted six weeks in advance, while others require that you submit them directly to the site versus turning them into Academic Services (T-301).
  3. Review Memorandum 41 if you are currently employed by a site you will also attend for a clinical rotation.

If the name of your site is not listed below, contact your preceptor to make sure you meet all of their requirements prior to the start of your rotation.
